On this page you will find a variety of resources to self manage your health and wellbeing.

We are working with our Patient Participation Group members to obtain their input to ensure that the content and layout provide the information, advice, and access to the relevant services that our patients need.

Health and Exercise Referral Programmes

Exercise can help manage, improve or reduce the chance of developing many health conditions.

Our colleagues at LED Leisure, have established a Community Health and Wellbeing team.   They work in the community to support local groups and organisations with their health and wellbeing, examples of these include Stroke Survivors activity sessions, Dementia Friendly groups supporting sessions by offering gentle mobility exercises as well as wellbeing walks.  Locally, LED Community Leisure runs a variety of different health programs to support people suffering with long term health conditions.

Alternatively, If you’re recovering from an illness or injury, or just want to improve overall health, you can request an Exercise on Prescription referral from your GP for a personalised 12-week exercise programme at Bridport Leisure Centre.

Social Prescribing Resources

Social Prescribers from the Jurassic Coast Primary Care Network (PCN) work alongside us to provide extra time, a listening ear, and practical support to help you live a healthier, happier life. Whether you want to manage a long-term condition more effectively or find support for life’s practical challenges, we are here to help you find the right path forward. 

The Social Prescribing team have collated a library of trusted local and national services that may help with health, wellbeing, social connection, and daily living.
