If you are planning a trip abroad and require any vaccinations –
You should Complete and Submit a Travel Questionnaire Two Calendar Months prior to your due departure date
Travel Questionnaire: you can complete and submit the form online or you can also obtain a form from reception
Click below for the online form
Once we have received your completed questionnaire, our practice nurse, Sally Cable will assess your needs and will advise you what vaccinations are required and what medications you may need. We will need to contact you to give this advice and to make necessary follow-up arrangements (schedule vaccinations/book appointments once everything has been signed off by your doctor).
Please ensure that your contact details are up-to-date and that you supply a day time/mobile telephone number.
These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Please also note that advice for malaria changes from time to time, so please visit this website so that you can keep up to date with the latest advice: Fit for Travel.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
